We visited the Spectrum Health Big Rapids Hospital on 21 November 2016 for an interview. We interviewed the staff and non-management areas such as the hospital capacity, departments in the hospital, and the hospital strategies (“Steps for conducting interviews,” 2016). They were very responsive and responded to all our inquiries. The hospital was found in 1926, and was earlier known as Mecosta County Hospital, joined the Spectrum Health system in 2013. It was in the same year that it was renamed to Big Rapids Hospital. The hospital has served the Mecosta County region and its environs through the provision of a full range of medical services including surgeries, acute care, and birthing among others. The hospital also provides a variety of home health care services, rehabilitation, and professional wellness services.
The hospital prides itself on serving its hometown through the provision of exceptional care (“Big Rapids Hospital | Spectrum Health,” 2016). According to the management, the hospital has been rapidly growing and advancing technically as well as technologically so as to serve better the community More critical care as well as single rooms have been added. In addition to this, they have acquired the latest diagnostic CT-Scans, and this is just the beginning of the hospital’s big ambition to be an excellent hospital. According to the hospital’s management, the Mecosta County region will continue enjoying the convenience of local expertise, coordinated care, as well as its expanded capabilities for the near future.

After the recent incorporation into the Spectrum Health system and the subsequent renaming, the hospital has been on a campaign to inform the community of its new look, new identity, new communication outlook, new examining rooms, through brochures, postures, and fliers. The hospital according to management remains to be loyal to its customer and promises to serve them better due to the increased capacity. From the interview, we determined that the hospital has sixty-nine beds and five hundred and fifty-three employees. After the incorporation into the Spectrum Health System, the hospital has considerably increased its capacity to handle patients. The number of admissions encountered per year as seen from the 2015’s figure has significantly improved. The inpatient’s admission in 2015 was 2,860 while those observed were six hundred and forty-seven. The number of outpatient cases was six thousand and eighty-seven while the number of emergency room cases in that particular year was 23,760. Finally, the ancillary cases observed during that given year was 89, 892.
The hospital has eleven health information employees who include one manager, one senior lead, nine analysts, and one ROI contracted employee. Their roles include chart assembly and analysis, birth certificate completion, and release of information and documentation. The staff, to effectively carry out their duties, are well educated (“Big Rapids Hospital | Spectrum Health,” 2016). From what we learned, the department manager holds an associate degree in Applied Science in Health information technology while some staff has a higher education that is outside of their field. The site manager of operations is in charge of HIM department and reports to the Director of HIM operations. He is responsible for the information analysis to facilitate compliance with the law, audit, and for billing purposes. Such information relates to the birth certificate and the records of patients in all the departments.
The HIM practitioner serves on various committees and is responsible for various roles in those committees. In the utilization review committee, he presents statistical information on the inpatient cases.he liaises with the facility and system and provides information to the DAT. He further participates in the community HIM to create consistency of process at each site. In the HIM community roundtable, he attends the meeting for ROI discussions and in revenue cycle, he reports any inconsistency with the information in chart review. Finally, his roles extend to the joint commission preparedness team where he ensures that the facility follows the joint commission standards.
The information from the interview reveals that the HIM department keeps various information and statistics that include the physician delinquency rates, admission and discharges, birth registry and ROI requests. In addition, there are numerous studies done as the HI manager continually creates and runs patients reports. It includes the valid IP status and Leapfrog survey. The facility has adopted computerized health information system such as the Meditech, Hyland on base, Hylad medical record management, Dictaphone and win scribe. Further, the service uses Cerner and Epic as electronic portions of the files.
Moreover, the employees had to train on how to use the electronic health record system according to the specific roles attached to them in the facility. Each department, therefore, generated their plan for its employee. The electronic health record system is beneficial to the hospital although there have been various challenges in its implementation. First, the system relies on the accuracy of users in making entries to the system. The information is reliable when the correct data entry is made into the system thus creating reliance over human beings. The Servers breakdowns and failures offer difficulties to the users forcing the employees to rely on the paper procedures. Moreover, there lack proper coordination and interaction among the system in sometimes thus making it dysfunctional. The information obtained in this interview was very useful, and we are glad for the cooperation and immediate feedback accorded by the hospital facility (Lichstein, 2016).
