For instance, a psychiatrist can help control the bipolar disorder that a psychologist can help with the drug abuse problem. The individual healthcare plan will be in effect once the healthcare provider, the student, and the family members agree to the formal written statement, concerning the treatment regimen.
The plan helps the school to obtain the information that can be used to provide special education for students dealing with given health conditions like the ones stated above. The student will use the information to avoid unwanted risks, to deal with stigma, and ensure they are always present for learning sessions. The plan helps the student to realize their academic, physical, social, and emotional objectives.

During the presentation before the nurse, the boy seems restless by moving from one place to another. This is unexpected for a 15-year-old boy, in a setting like this. The mother has explained that the boy has been expelled from two schools due to his deteriorating behaviors. The boy has a tendency of forgetting his medication if not placed strategically. Another element of his behavior that should concern the nurse is the acting out that his mother reported since she started attending the new working place. The form presentation observation could be linked with the signs and symptoms of bipolar disorder and drug abuse problem as shown in (“Bipolar Disorder in Adults.”….. Para 1-5). For instance, bipolar patients are characterized by a shift of behavior from the regular expected to antisocial behaviors which affects relationships negatively. This is typical with bot being expelled from two schools linked to his behaviors. The changing of sitting positions from one point to another within a short duration is a sign of being restless. Restlessness is a typical characteristic of drug abuse patients as indicated in (Todd and Heidi p.503). With all those behaviors and their connection to his health problems, the nurse should take note of those behaviors to trace their origin.
