Section 1: Question 1
Despite the progress that healthcare providers have made in the quest to push for coordinated care, integration of the various health systems remains a major setback. One of the top barriers is that there lacks a standardized approach of assigning patients all over the country a unique identity. The primary motive is to enhance coordination among healthcare providers. There are no precise interoperability standards that can be used across multiple venues of healthcare (Weaver et al., 2016). The local, state and federal government should unite with other stakeholders in the healthcare docket to create a national identifier system where every patient can be assigned a unique identity.
Additionally, some of the health systems in place may lack a common language through which they can share information because they were designed for use in different settings. Medical practitioners should establish software and hardware standardizations that should be used in the various health information systems to facilitate interoperability (Weaver et al., 2016). Data sharing is prone to incidents of security breaches to the point that some health facilities have intentionally declined to share data regarding their patients. Health IT developers should strive to ensure that the network is secure to avoid data breaches or unauthorized access during information sharing.
Section 2: Question 1
All the key players and stakeholders in the healthcare docket should be involved when redesigning workflow.

One of the current obstacles is that most physicians and nurses are not trained in how the healthcare systems operate. All the frontline staff in every clinical setting should be equipped with skills of how the systems operate (Singh et al., 2013). Patients, on the other hand, should be emancipated on the need to cooperate with their healthcare providers in the quest to facilitate redesigning of the workflow. In a nutshell, both the physicians and patients are finding it challenging to adapt to the new changes in the healthcare docket. Policymakers should intervene to ensure that all parties are trained on the operation of the systems and the need to cooperate with the healthcare provider.
