Kate is a 42-year-old woman who is married with one daughter away in college. She is complaining of a severe depression that has lasted for at least 12 years starting with the death of her mother. The client appeared anxious and irritable with the manager during the interview as a result of her depression and unrest. She seems to have impaired occupational and social functioning that is accompanied by hypersomnia. The condition is discouraging to her such that she skips several working days in a month. According to the client, she has lost interest in participating in social activities as well as making good relationships with her friends. The current situation in her family makes her seem heartbroken. However, Kate should do something positive to restore her attitude so that she can live a comfortable life.
Firstly, the client should seek for psychiatric and medical examination of her depression for at least six sessions. The advice should involve that involve individual advice regarding how she can personally persevere through the challenges. Through consultation and medical visits, Kate is unlikely to commit suicide. She will receive quality mentorship concerning how to withstand the problems (Assessment, 2008). Kate cannot readily stand for herself. She deserves external support from both her family and other well-wishers. Kate has several strengths and opportunities in her life. According to the client, she has an area to work and hence she can earn a living.

Marriages and Significant Relationships currently: Carlos is not married to any wife. It is not known when he is likely to marry.
Birth and Childhood: Carlos remained in Mexico at a young age when his family migrated to united state. He lived with his aunt for quite some years. He was difficult to handle, and hence his family left him with his aunt.
Military Service There was no military service.
Education: Carols has no education achievement. It was not easy for his parents to control him. Therefore, he did not attend a school as his brother did. He remained with his aunt in Mexico for more than ten years. In the process of their stay, his aunt passed on, and hence he went to join his family abroad.
Current Living Arrangements: Carlos lives with his family in United States. He is unable to cope with his emotions challenges concerning the new country. He is not sure whether he should continue staying in the US or return to Mexico. According to the client, he is demanding to quit and go back to Mexican or any other Place where he will find peace.

Besides, the cultures of the US create discomfort to him. Therefore, Carlos should attend guidance for at least three times in a week so that he can learn how to adjust to new culture settings.

According to the client, his grandfather is unable to receive sufficient medication as a result of poverty in their family.
Marriages and Significant Relationships: john is not married, but he plans to wed in the next four years. However, the client says that he can only manage to marry if he can secure a job within the first two years after he graduates.
Current Living Arrangements: John and his grandfather live in a single bedroom home in California. While John is in school, the old man is left to live alone. John says that if he is able, he can relocate his grandfather to a place where medical facilities are available.
Employment History Right: John works with beauty and Textile Company when he vacates for holiday sessions. He also works with studio productions where he trains other youths on how to create and produce sings.

Education: John is a current undergraduate student at California University doing a degree in mass communication. His performances in the university are not satisfying due to fees challenges.

Every agency has its ways of recording the social history of people. However, the method of recording depends on reliability and availability of the resources.
