Grade: ThreeTheo is a seven-year-old boy who studies at Northampton Elementary School. He has been receiving occupational therapy to improve his handwriting and ability to pay attention. Theo appears to have made significant progress within the two areas of concern. The child demonstrates a good pencil grasp as he applies less pressure when holding the pencil. As a result, he is able to form all letters correctly. In particular, Theo can form letters in appropriate sizes, within lines, and he does not have a problem with aligning characters along the baseline.
Theo can sit still and pay attention in class without disturbing other children. He can also follow instructions both at home and in school. At the same, Theo demonstrates proper pre-writing skills and he can now draw squares and triangles without slanting or extending them. This can also be confirmed by the fact that he no longer has a problem writing alphabets and letters. Furthermore, Theo rarely gets distracted in class and this allows him to focus more on studies.
In this light, Theo has achieved all the goals written in his IEP. He can now attend school without getting into trouble everytime. At the same time, he can be in a position to keep up with the classroom demands such as sitting still, keeping calm even during long sessions and following directions.

His handwriting has also developed, and therefore it is expected that his academic performance will improve as well. More so, at home, the mother has an easy time with Theo as homework is done with ease. Therefore, discharge is advised since Theo no longer needs skilled services in order to pay attention and write well.
