The patient’s age, gender, and ethnicity are some of the factors that have effects on the risk factors associated with the health of the patient. In the given case, MK is an African-American lady who is seventy years old. The patient was experiencing episodes of confusion and dizziness despite in the past been diagnosed with hypertension, chronic renal failure, asthma, type II diabetes, breast cancer. The age of the patient is quite advanced making her prone to different forms of illnesses. The gender of MK is also a factor that is likely to have triggered or accelerated the illness. Being a female, MK had undergone a C – section that made her vulnerable to those conditions. The ethnicity of MK is also another factor that is likely to have contributed to the worsening of the condition (Sidani, 2011).

Modifiable factors are referred as the risk factors that can be controlled or maintained. Additionally, the effects of the risks can be reduced through the modification. There are many modifiable factors that can be associated with the patient. For instance, MK is overweight, having a weight of 176 lbs which is a modifiable factor that has made her vulnerable to the condition. MK has diabetes which is associated with the daily uptake of insulin. This condition can be modified to reduce the effects of the condition of the patient. The lack of physical activity is another modifiable factor that seeks to have effects on the condition of MK.

The blood pressure of MK is high which is recorded to be at 172/94. The same increase in the blood pressure can be attributed to higher cholesterol levels in the blood of the MK. MK has initially been diagnosed with a set of conditions that serves as the basis for conducting the diagnosis to the patient. The conducting of the primary diagnosis is a paramount aspect of nursing because it paves the way for the principal diagnosis, which establishes the condition that causes the condition that is being experienced by the MK. The history of smoking for twenty years, different surgeries that she has undergone, as well as procedures such as radiation that have been conducted for over a period of four years. The nurses will use this information including the medication that MK is taking to assist the doctors in conducting the principle diagnosis to ensure that the condition affecting MK has been addressed (Holloway, 2013).

The physical finding that have been presented regarding MK offer a clear confirmation of the admitting diagnosis. Apart from the episodes of dizziness and confusion that MK has been experiencing, it is also clear that there are other many underlying conditions that might have contributed to the worsening of the condition. For instance, the removal of her lesser toes is one of the physical findings that might have impacted the condition of the patient as well as causing the situation to be bad. Other conditions such as asthma, chronic renal failure, hypertension, and breast cancer are other physical results that reveal that her body is relatively weak to handle the condition. Psychological tests are expected to be made a priority in addressing her condition. Another test that can be used to address the condition that MK is suffering from is determining the current condition of the kidneys as well as that one for the breast cancer. These tests are expected to be conducted consistently to make sure that all issues have been addressed within the stated time so as to allowed the patients to start receiving the required and the desired medication (Chitty, 2011).

MK has many signs that reveal that they are out of the normal range. The weight of MK is way above the expected range of 114-149 for individuals at that particular age. The issue of overweight needs to be addressed. The other vital sign that is present in the diagnosis of the blood pressure is out of the normal range that is expected for individuals of that particular age. The expected blood pressure for individuals, a female of 70 years of age is 120/80. The expected and perfect range for the systolic pressure should range between 120 to 139. The range that is expected for MK to be within the normal range is 80 to 89 diastolic range. The other thing that appears to be out of the normal is the medication intake that MK has exhibited especially for the insulin. The uptake is supposed to be harmonized so as to ensure that she does not overdose or overdose which is a factor that might worsen her condition. The choice of the inhaler is also supposed to be one unlike having had obtained different information from her doctor. The primary diagnosis is expected to highlight on the abnormal vital and give them priority these conditions while seeking medication (Butcher, 2013).

The major medication that should be prescribed to MK is Cholinesterase Inhibitors as galantamine, donepezil, and rivastigmine is crucial means of addressing the mental condition of the patient. The mental problem experienced such as the loss of memory can be addressed by these medications in efforts to mediate the condition. The major side effects that are associated with this form of medication is weight loss, vomiting, nausea, and diarrhea. The test that of great importance for the medication is the psychological tests and psychiatric intervention (Holloway, 2013).

The educational plan that I would create for this patient will include PowerPoint presentation, learning the material, outpatient care, and Videos. The plan will ensure that the patients will be undertaking various forms of therapies to stay fit. Home care nurse will be visiting the family regularly to monitor her progress and assess the changes needed in the medication. The family members will be included in this plan because they are expected to work in collaboration with the home caregivers for the success of the treatment of the patient. They will also be assisting MK with the educational materials and help her implement the plan fully (Sidani, 2011).
